What is your ePO version ?
Have you tried downloading the package and check-in manually in Extension page.

Check this KB to see if all the versions match - maybe it got partially installed. I would also highly suggest opening a ticket please.

Receiving the same error when trying to install extension manually. Any update on this issue?
@Pikapp691 I would recommend opening a ticket. There could be different reasons for the failure and we would need to see logs and possibly get sql information.
